Fallen Empires is famous for its extensive use of counters and tokens. In order to improve playability, a sheet of double-sided cardboard counters and tokens was inserted in issue #4 of the The Duelist magazine in March 1995.
Despite being customized for Fallen Empires cards, most counters and tokens can be used for other cards as well.

The counters and tokens are not equally divided on the sheet. Their exact frequencies are:

-1 / -1 Counter: Use for Power and toughness alteration. Placed on Creatures. Introduced with Unstable Mutation in Arabian Nights, Revised. It was used with Thelon's Chant, Tourach's Chant in Fallen Empires.

-1 / -0 Counter: No cards from Fallen Empires associated with it at all.

-0 / -1 Counter: Introduced with Lesser Werewolf, Takklemaggot in Legends. No cards from Fallen Empires associated with it at all.

+0 / +1 Counter: Introduced with Living Armor, Necropolis in The Dark. Originally, Living Armor from The Dark put a +0/+X counter on a creature. It was given errata to place X +0 / +1 counters on instead.
It was used with Dwarven Armorer in Fallen Empires.

+1 /- 1 Counter: No cards from Fallen Empires associated with it at all.

+1 /+ 0 Counter: Introduced with Clockwork Beast in ABU, Revised. Clockwork Avian in Antiquities. It was used with Dwarven Armorer, Ebon Praetor in Fallen Empires.

+1 / +1 Counter: Use for Power and toughness alteration. Placed on Creatures. First introduced with Fungusaur, Sengir Vampire in ABU, Revised. Khabál Ghoulin Arabian Nights.

Odyssey may have pushed token creatures to the forefront of tournament Magic, but the biggest set for tokens (and counters) was probably Fallen Empires from 1994.
There were so many cards that produced tokens and/or required counters that we issued a cardboard sheet of them in Duelist #4.

Included on the sheet were 0/1 Thrull tokens (for Breeding Pit), 1/1 Camarid tokens (for Homarid Spawning Bed), 1/1 Saproling tokens (for the myriad Thallids), 1/1 Goblin tokens (for Goblin Warrens), and 1/1 Citizen tokens (for Icatian Town).

To make bookkeeping even more of a headache, the sheet also had Time Counters (for Tourach's Gate), Net Counters (for Merseine), Javelin Counters (for Icatian Javelineers), Credit Counters (for Icatian Moneychanger),
Tide Counters (for Homarid), and Spore Counters (for all the Thallids).
But wait, there's more! The sheet also had +0/+1, +1/+0, +1/+1, +1/+2, +1/-1, +2/+1, +2/+2, -0/-1, -1/-0, -1/-1, and -2/-2 counters that worked with everything from Dwarven Armorer to Ebon Praetor.
Some of which had no relevant cards associated with them at all.
