I don't recall of ever seeing any in a sealed pack of one. That would mean that WotC would have to send a pile of packs to each TO just for one FNM month. when I was a TO all the promos came in larger packs.
I do remember player reward and the special foils for Fat Packs having single cards.

This is real old news, they were mentioned on the forum, and a bunch of them were available back in the years either on The Bay either in MTG Shops in the US, or from private collectors. There should be topics somewhere in one of the subsections of the forum.
