I like the collection of stuff, although I cannot tell what is wrong with the last few! Hi,
This effect is caused by dipping a q-tip into some nail polish remover and carefully scraping the head of the q-tip against the foil portion that you wish to remove.

My misprint gallery!

I am sure that this is miscut into cardstock that has no template (but it may not be a filler). I'm sorry that no one got back to you sooner.
All foil Walking Sponges have that error. Most collectors don't consider this a worthy misprint, and certainly not a valuable one, since it has an occurance of 100%.

They are very rare. However, I have seen many CHK with squares, so I believe it to be more common there than in other sets. Cards can have any number of squared corners.
